Osteoarthritis is a condition that causes problems of wearing out of the normal smooth cartilage surfaces of the joints. Often called wear-and-tear arthritis, osteoarthritis causes joint pain, swelling, and deformity. Osteoarthritis is the most common type of arthritis.

How do glucosamine and chondroitin work?
Glucosamine and chondroitin are two molecules that make up the type of cartilage found within joints. Inside your joints, cartilage undergoes a constant process of breakdown and repair. However, to be properly repaired, the building blocks of cartilage must be present and available. The theory behind using the glucosamine and chondroitin joint supplements is that more of the cartilage building blocks will be available for cartilage repair.

  • Glucosamine is a precursor to a molecule called a glycosaminoglycan-this molecule is used in the formation and repair of cartilage.
  • Chondroitin is the most abundant glycosaminoglycan in cartilage and is responsible for the resiliency of cartilage.

Treatment with these joint supplements is based on the theory that oral consumption of glucosamine and chondroitin may increase the rate of formation of new cartilage by providing more of the necessary building blocks.

What research has investigated glucosamine and chondroitin?
There have been numerous studies to examine the treatment effects of glucosamine and chondroitin over short periods of time. Most of these studies last only one to two months; however, they have indicated that patients experienced more pain reduction when taking glucosamine and chondroitin than patients receiving a placebo. The improvement experienced by these patients was similar to improvements experienced by patients taking nonsteroidal anti-inflamatory drugs (NSAIDs) that have been a mainstay of non operative treatments. The difference is that NSAIDs carry an increased risk of side effects including gastrointestinal complaints and bleeding.

The joint supplements glucosamine and chondroitin have virtually no side effects, and thus their use would be preferred if the effects of pain reduction were indeed the same. Furthermore, there is an indication that NSAIDs may increase the progression of arthritis, whereas glucosamine and chondroitin generally offer a more protective effect to the cartilage surface

In one recent study, the effects of chondroitin in 372 patients over a minimum three month period was investigated. This study, which combined the data from other smaller studies, found that patients did have an improvement in their symptoms of pain.

There have also been long-term studies lasting over one year into the effects of glucosamine. These studies also have shown encouraging results. A presentation at the 1999 meeting of the American College of Rheumatology illustrated the findings of a three year long European study that compared the effects of glucosamine versus placebo in about 200 patients. The group of patients taking glucosamine had less pain and better x-ray findings than the group of patients taking the placebo.

Glucosamine—An Amino Sugar

Glucosamine is an amino sugar—a molecule made from an amino acid and a simple sugar. Amino sugars are the basis of virtually all connective tissues and lubricating fluids in the body. Just as amino acids are the building blocks of proteins, amino sugars are the building blocks of giant molecules called glycosaminoglycans (GAG’s), also known as proteoglycans and mucopolysaccharides. GAG’s are large, spongy, water-holding molecules that form the glue that holds us together. This substance is found in all connective tissue and mucous membranes. Numerous double-blind, placebo-controlled studies have examined the positive effects of oral administration of 1,500 mg of glucosamine sulfate-the amount in one daily use of  JOINT COMPLEX. .

Chondroitin Sulfate

Chondroitin sulfate is the most abundant GAG in the body. Its main role is in keeping cartilage fluid and elastic. It is found naturally in the body, where it is one of the critical compounds that makes up connective tissue. Connective tissue is responsible for building and supporting cartilage found in the joints and elsewhere.

Dietary Sulfur for Joint Lubrication

Shark Cartilage

Researchers investigating the use of shark cartilage to treat cancer also experimented with arthritis patients. Several clinical trials have proven the effectiveness of shark cartilage in treating arthritis.

In one study conducted in early 1970s, arthritis patients suffering from severe pain were given cartilage injections. Following three to eight weeks of cartilage injections, of the 28 patients in the study, 19 reported "excellent" improvement, six rated their improvement as being "good" and three did not respond to the treatment. Of more importance was the observation that the relief lasted from six weeks to more than a year, and there were no toxic effects.

Another study involved 10 patients bedridden with osteoarthritis. Following three weeks of receiving shark cartilage orally, eight of the 10 were able to leave their beds and move around with relative ease.

In a larger-scale study, 147 patients were given either cartilage extract or a placebo. Those who received the placebo were also encouraged to use various medicines when their pain flared up. Five years later, the cartilage group reported a drop of 85 percent in pain scores, compared to a 5-percent drop in the placebo group. The cartilage group also had less joint deterioration and lost less time from work due to pain.

The results from these and other clinical trials suggest that shark cartilage may help to reduce or relieve pain in 60 percent or more of those suffering from osteoarthritis and rheumatoid arthritis. People with other forms of arthritis have also benefited.

One should begin to notice a lessening of pain and/or swelling within two to three weeks after beginning the shark cartilage. In cases where symptoms are not reduced within four to five weeks, the cartilage will probably not be effective.

Beta Sitosterol

In a randomized, double-blind, placebo-controlled, multicenter study of 200 men with benign prostate enlargement, half the group received 180 mg of beta-sitosterol daily, while the other half received placebo. After six months, the beta-sitosterol group saw improvement in the International Prostate Symptom Score, the measurement of urine flow (QMax), and the amount of residual urine remaining in the bladder.




The beta-sitosterol group showed a 7.4-point decrease in the International Prostate Symptom Score, compared to a decrease of only 2.1 points in the placebo group . . . a significant 3.5-fold improvement in the men taking beta-sitosterol .

The measurement of urinary flow increased to an average of 15.2 milliliters (ml) per second from 9.9 ml/second in the men receiving beta-sitosterol. The placebo group only increased to 11.4 ml/second from 10.2 ml/second at baseline. Urinary flow thus improved almost 35% in the group taking beta-sitosterol, compared to only 11% in the placebo group.

Most remarkably, residual urine in the bladder decreased to 30.4 ml from 65.8 ml in the men using beta-sitosterol . . . a reduction of almost 54%! In the placebo group, residual bladder urine declined from 64.8 ml to 54.3 ml . . . a reduction of only around 16%.3

In a follow-up study that evaluated durability of response to beta-sitosterol, the beneficial effects for beta-sitosterol were found to be maintained during an additional 18 months of observation.
